Where it goes

The certificate is “forwarded to the Engineer”. The form prints no address.

the City of North Vancouver publishes no form, no cross connection control page and no intake address. The duty to file exists; the destination does not. If you have been told where to send it, we would like to know — see below.

How often, and by when

Annually, in July. Not stated in the bylaw — confirm with the authority. We would rather tell you we did not find one than invent a number you might plan around.

Worth knowing

The due date is a FIXED CALENDAR MONTH, not an anniversary. A device tested in March is still due the following July, not the following March.
