Filing a backflow test report to the City of Salmon Arm

Where it goes

Email, fax or post to the Cross Connection Control Program Coordinator.

How often, and by when

Annually. The report is due within 30 days of the test date.

Worth knowing

Submitting IS the attestation — the form carries no signature line. The City also reserves the right to stop recognising a tester who repeatedly files forms with errors or omissions.
