The bylaw requires a report; no intake address or form was found.
Fort St. John publishes no form, no cross connection control page and no intake address. The duty to file exists; the destination does not. If you have been told where to send it, we would like to know — see below.
How often, and by when
Upon installation, annually, and on request. The report is due within 30 days — the bylaw does not say from what event.

Whatever the deadline, the report only stands if your test kit was in calibration on the day. Most programmes require it every twelve months, and a lapsed kit does not slow you down — it stops you filing at all. We will email you before yours lapses, free.
