The District's test report form says it can be emailed to permits@westvancouver.ca, the permits desk.
How often, and by when
Not stated by the District. Not stated in the bylaw — confirm with the authority. We would rather tell you we did not find one than invent a number you might plan around.

Whatever the deadline, the report only stands if your test kit was in calibration on the day. Most programmes require it every twelve months, and a lapsed kit does not slow you down — it stops you filing at all. We will email you before yours lapses, free.
Worth knowing
The waterworks bylaw has no backflow or cross-connection provisions; the building bylaw requires a permit for backflow preventers in lawn sprinkler systems. West Vancouver is not the City of Vancouver.
